At the heart of the Guardian Partner program is an Advisory Council made up of the community’s leading physicians, nurses, clergy, attorneys and nursing home administrators who base their recommendations and decisions upon the life-long values, beliefs and preferences of each individual.

The Application Process

You can refer a potential client to the Guardian Partner program. Clients who are accepted must have a healthcare professional who can attest to their mental incapacitation. Upon approval, REAL Services will petition the court for a hearing and prepare necessary paperwork. Applicants must be:

Sixty years of age or older
Determined to be mentally incapacitated or unable to make sound decisions for themselves
Have no other willing, able or suitable guardianship options available
